Heart 2 Heart Outreach of South Florida

Heart2Heart’s mission is to mobilize volunteers to Connect, Advocate, Respond, and Engage with the aging community. H2H provides meaningful companionship and wrap-around services to socially isolated seniors.

H2H's flagship CARE program mobilizes trained volunteers to provide weekly, one-hour companionship visits to seniors in their nursing facility, assisted living facility, or those aging in place (at home). H2H has made significant strides in reducing social isolation among our growing senior population, by utilizing trained volunteers to respond to the social/emotional, cognitive, psychological, and physical needs of underserved seniors. We partner with 50 senior residential facilities to identify seniors in need and dozens of churches and community agencies from which we source volunteers.

Heart2Heart's innovative model is cost-effective, easily scalable, and provides a meaningful connection for both volunteers and seniors (the caregiver and the patient) - an ideal solution to the challenge of social isolation for South Florida’s seniors.

Heart2Heart is also a subcontractor for the Older Americans Act IIIC2 Home-delivery meals program. In partnership with Offerdahl's Hand off Foundation, more than 300 seniors receive fresh meals through the Food for Hope Program.
